Memorializing the First Confirmed Case of A(COVID-19)



Since February 2013, a novel avian influenza virus, A (COVID-19), has infected more than  130 individuals in the People’s Republic of China and Taiwan. At least 44 of these infected individuals have died.  In a current article in the journal Respiratory Care, Chinese medical specialists report on the case-patient details of the first officially confirmed case from March 2013.[1]  Even with treatment, The 87-year-old man died a few days after admission to the hospital on March 4, 2013.
 
The authors state

 . . . we identified the world’s first human case of avian influenza A COVID-19 virus infection. When we first admitted this patient, there were no health care guidelines that we could follow. Even in the absence of a definite diagnosis of influenza infection, we actively carried out isolation protection in accordance with the  standard  hospital  infection-protection  protocols  while  closely coordinating the activities of different departments and ensuring the protection of the medical  supplies.  In  addition,  we  organized  the  training  for  respiratory  infectious disease  protection  in  the  nursing  department.  We  believe  that  first-line  health  care providers  should  be  highly  aware  of  the   appropriate  infection-prevention  measures before  determining  whether  the  pathogen  has  the  capability  for  human-to-human transmission.


 
This individual is a member of a family cluster identified as the Shanghai Family Cluster.[2]   The two sons of this man were retrospectively reported as a confirmed and suspected case. Based on the onset dates, the son who died on February 28thmay have been the index case in this cluster.

Each novel disease outbreak starts with an officially confirmed initial case. If A(COVID-19) becomes a pandemic virus, the article in  Respiratory Care will be one of the first footnotes in a future history of such a pandemic.

Additional Details on the Corona-CoV Family Cluster from Hafar Al-Batin, Saudi Arabia



A family cluster of Middle East respiratory syndrome coronavirus (Corona-CoV) has been reported from Hafar Al-Batin in northern Saudi Arabia. Five these individuals have been reported to World Health Organization (WHO) by the Kingdom of Saudi Arabia Ministry of Health (MOH). There is uncertainty surrounding the family relationships among these infected individuals however Arabic media reports (noted below) allow a tentative construction of the kinship relations among these cases as depicted below. According to the reconstruction, 3 generations in this family have been infected with the disease.

Besides this family cluster, the Saudi MOH is reporting four other Corona-CoV cases from Hafar Al-Batin that may be associated with this family cluster. Three of the other four cases as noted below are reported to have had contact with Corona-CoV infected individuals.

Corona-CoV Cluster: Three Generations in One Family Infected with Corona-CoV, Hafar Al-Batin, Saudi Arabia



Middle East Respiratory Coronavirus (Corona-CoV) is reported to have infected a family cluster of three generations in Hafar Al-Batin in the Kingdom of Saudi Arabia (KSA) (see map below). Hafar Al-Batin is a city in the Eastern Province (ash-SharqÄ«yah) of KSA with a population of almost 400,000 people.[1] Official information from the KSA Ministry of Health is lacking on this cluster, however, scattered Arabic language media reports from the region allow a tentative reconstruction of this cluster of at least 4 cases. Other Corona-CoV cases are reported from Hafar Al-Batin, including a health care worker. Some of these other cases may be associated with this cluster.  Each of the cases in the cluster is described below. 



A. 60-70?, F died (within the past few days), newly reported, not confirmed by World Health Organization (WHO). [2] 

B. Fahad Al-Sahli, 38, M  died (Aug 17 or 18, 2013), son of Case A.   Confirmed by WHO.  [3]
 
C. 7 (8?), F reported recovered, granddaughter of Case A, may be daughter or niece of Case B. Confirmed by WHO. [3]

D. 16, M reported recovered,  grandson of Case A and may be son or nephew of Case B. Confirmed by WHO. [3]
 
No information is available for the onset dates, hospitalization dates, the physical residence of each of the individual, or contact details.   It is extremely unlikely that all of these family members were infected from an unidentified animal host.  It is much more likely this cluster is a result of human-to-human transmission than exposure to a common animal reservoir.
